I must apologize to all my wonderful patients. We are having some issues with our carrier. If you ever have trouble contacting us, always go to our answering service. They have all the information if there is an issue. Their direct number is 239-210-6042.

You can also go to your patient portal and send us a message at any time.

Another option is to fax us: 239-415-1136.

Lastly, you can email us: [email protected].

We are working as quickly as possible to fix this, but always use any of the above options.
